We are joined by June Simmons, CEO and Founding President of Partners in Care Foundation (Partners). For over two decades, Partners has been a social care innovator leading the nation in designing strategies to align social and health services for vulnerable populations. Partners’ HomeMeds℠ software is an evidence-based medication inventory program used by organizations and pharmacists nationally to combat and eliminate medication related errors in the home for older adults. June Simmons has been innovating in the use of pharmacists in home based care for over two decades. Maybe before it was really cool. :) Her organization, The Partners in Care Foundation, uses pharmacists in combination with home meds making a difference in eliminating medication related errors. A forerunner of this work, June's compelling story in medication management is one not to be missed.

Times have changed and the informational age is now becoming the artificial intelligence age.
